Prepping old subfloor for hardwood

Most of our house is 2.25” oak hardwood. We are renovating the kitchen and living room and preparing to lay hardwood to match the rest of the house. After tearing up the carpet and terrible particleboard that was ring-shank nailed to the subfloor in the living room, this is what the old subfloor looks like. They’re about 3/4” x 7 1/4” boards laying horizontally with a couple almost 1” gaps. They’re nailed to the floor joists.
What do we need to do to prepare it for hardwood?
Anything more than repair some of the boards, lay vapor barrier and match the existing wood in our house?
