newbie NPK question (cross-posted to soil forum)

I've read the FAQ and did a search and I'm still confused.

I have a garden area that's had a few perennial herbs, some mostly decomposed wood chips, and lots of weeds growing in it. I want to consolidate the herbs and grow vegetables in a no-till, organic fashion.

I got a soil test, and the NPK part of the test has ratings of very low, low, medium, and high. My results were N=very low, P=medium, K=very low.

The soil is black, crumbly, and wormy. Can it really be that low in nutrients when it LOOKS like good soil? (I vaguely remember reading something long ago about wood chips being bad because they steal nitrogen?)

I have seeds that I was hoping to plant TODAY but now that I've done the soil test, it seems that might be futile. I don't know what to do about my soil. Any advice? Can I plant now and topdress or mulch to improve the soil as the plants are growing, or will that not get nutrients to the roots?
