Encapsulating a crawl soace

We are looking into getting our crawlspace encapsulated. It currently is very damp and has a bit of condensation. The house was built in 2006 and we live in Southern NC with hot and humid Summers . Currently it only has a plastic moisture barrier on part of the ground inside and we've known we wanted to do this since we bought.. I'm reading a lot of discussion boards saying what you should/shouldn't do and I'm hoping to get more straight forward answers through my own post.
When we have this done, should we be sealing the vents?
Should we have them use foam or the plastic material?
Is a dehumidifier necessary? (we've had
the one company bring this up already)
Is there an a rate I should be comparing per sq. foot?
What should I look for in a contractor/company who does this?
While shopping for contractors, what should I be asking?
