patchy lawn, crappy soil, lots of weeds..

Just started caring about the lawn. Got a soil test and showed low nutrients and low pH (4.5 in some spots 5.1 in others). It's been very patchy and grass is hard to grow. Applied some lime last year but it's time to kick things into high gear this year. My question is more along the lines of order of operations:

Do I spread seed even though the soil quality is poor, or should i work on getting the soil up to par first?

Should I lime and fertilize (i have the soil tests, so i know what nutrient ratios to target) a few times before putting new seed down?

what about a pre-emergent herbicide? Should i put this down, and if so that means i need to wait a long time before seeding again, right? We usually get lots of crabgrass
