Help! Total newbie trying to save dying succulents!

I have a 400 W LED grow lamp suspended about 3 feet above my succulent arrangement. The plants are kept indoors with temperatures generally above 60 and never higher than 75 degrees Fahrenheit. The soil is a 2:3 mixture of potting soil to perlite. The plants are spaced least 3 inches apart from each other, and receive 16 hours of light per day. The leaves are turning yellow, wilting, and falling off, and I'm very concerned about the health of these plants. Just purchased from a nursery a week ago, and my Etekcity soil moisture meter is reading that the soil is just barely above the "Dry" range, which puts it in the "Moist" range. I've attached pictures, and I would appreciate any and all advice on how to prevent the death of these beautiful plants!
