casing and baseboard remodel

our house is about 10 years old, and the baseboard and casing has been beaten pretty good by the previous owners. my current plan was to replace the baseboard with 5 inch (currently 4) and redo the window and door casing to be true 3 1/2 inch all around instead of the current top "decorative" piece. I planned on a complete redo with mitered corners all around, but some of the door casings are cut down to fit crappy design. basically looking for alternatives to mitered corners, would a new top piece on a butt joint work? or should I just leave them as is? to me the top feels a little dated and about as barebones as you can get, anyone else have any luck redoing these?
