Desperate about dwarf variegated Eureka Lemon tree

I have had this tree fro 15 years. That was the last time I saw fruit on the tree. It lived in a huge concrete pot in a terracotta color disguise. It had been blooming for 15 years. As soon as small fruit formed, it blackened and fell off. Leaves were always abundant and healthy. I was told that the concrete may leach a chemical in the soil and that is the reason for the fruit falling off. This year I transplanted the tree in a 25 gal plastic pot and hoped for the best. OMG! It got worse! The leaves are falling off; the fruit does not blacken any more but falls off after it is pea size. I am using a moisture meter, sometimes dig my hand in 10-12 inches to check for moisture; feeding the plant. I want lemons! Any ideas I should try?
