Pittsburgh Lawn with Disease / Fungus / ? Struggling for years

Hi all, I'm in Pittsburgh PA and I've been struggling with this lawn for a few years. My usual routine has been 4 applications of Milorganite per year. This year I aerated in early September, over seeded and applied pelletized lime based on a PennState soil sample I did in 2016.

I can share more of the application history, but basically: Nothing seems to be working. I can pull clumps of grass up in most areas, there is no deep rooting of my lawn anywhere. I folded back a large section looking for grubs (and even did a grub application this fall) due to the roots & seeing holes, but the holes seemed to be due to squirrels burying acorns. No dead grubs after application.

This looks like some sort of disease, but the whole lawn has this appearance (approximately 7000 sq. ft). I know this is a bit of a rant, but I'm a bit at wits end. Does this look familiar to anyone? I can take additional pictures
