Fermented Dills question

I am planning to start a batch of these tonight but have questions....

1. The directions say to rinse, but not wash the cukes. I usually rinse with cold water and rub all the little knobbies off. Is this OK?

2. There's no mention of trimming the blossom end (or both) before putting them into the gallon jar. Should I?

3. Boiling water? Warm water? Cold water? Does it matter?
