I Got My Soil Test Results Back. What Do the Numbers Mean?

I tested the heavy clay soil at the construction site of my new home. I sent the soil samples to my County Agent. The County Agent sent me a very confusing printout with my soil results.

As near as I can tell, this is what the results are:

FRONT YARD

pH = 4.8

P = 45

K = 116

CA = 894

MG = 387

BACK YARD

pH = 4.9

P = 42

K = 113

CA = 1318

MG = 303

So what does all this mean? What do I need to do to my yard to make the soil better for planting perennial flowers?
