Got my soil test results - Need some help/info

Growing Tifway 419 Hybrid Bermuda. Yard size is about 2000sqf.

Got my soil test results back today and this is what it says:

pH = 6.2

P = 31 lb/acre

K = 358 lb/acre

The only information I could fine regarding what is right for Tifway 419 was this:

"Tifway grows best when soil pH is between 5.5 to 7.0. Soil levels of 100 pounds/acre (112 kg/hectare) of phosphorous and 150 to 200 pounds/acre to 224 kg/hectare) potassium will provide sufficient plant growth. Nitrogen applied at 1 pound/1000 sq. ft. per month will provide a healthy plant when combined with the recommended phosphorous and potassium."

I went to the local outpost and showed them the test results and I was told I needed to raise the pH slightly so they sold me 2 50lb bags of lime. They also sold me a few small bags of SuperPhosphate (0-14-0) and a large bag of 34-0-0 Nitrogen.

I was told to spread all of the lime, all of the phosphate and then spread 6 lbs of the Nitrogen every 1-2 months.

So I have 2 questions...

1. I have a Scotts 2000 spreader. How do I know what setting to use when spreading this stuff?

2. Does the test results/info/advice Ive been given all seem correct?
