I don’t think you need anything on the walls. Maybe a floor lamp or a tall plant behind the chair on the left. Other than that, your room is nearly chockful of stuff. Mainly furniture.

Maybe consider painting the tile surround black to balance with the TV, then a nice fireplace screen in front. I would then add molding to extend the fireplace up to the ceiling.

What about large art on each side of fireplace? A DIY that might help balance the TV is to paint either the mantle or the tile surround black. You can search Houzz photos for inspiration. Kinda like this:

the walls are two stories (hard to see in the picture) so not sure if extending molding up would work. also the blck consoles can be moved elsewhere.

It's such a great wall space that is completely underutilized. (and that huge chair on the left side needs a diff home!)

let me show you some things I've done, you see if you like anything.

This one doesn't involve built ins. Just redo the surround, get some consoles or chests for either side, you could hang art or mirrors above them. I'd also bring in a nice chandelier. (can't tell if you have one)

With a tall ceiling, you don't have to do tile or stone all the way up, you can do simple mill work like this trim. it's easy, looks great, and lets the eye continue upward. A simple black tile surround

Just look at the trimwork, it can be done in a darker color like this shiplap all the way up. Floating shelves on the sides, a nice light.

And that tile on the fireplace has had it's day. I would strongly suggest you completely makeover that fireplace.

oh, last one. simple, generic built ins w/a common slate bar style surround. you could get simple taller cabinets like these, paint the inside a dark color, white exterior.

Have you considered moving the sectional along the window wall and putting the tv on the table with the lamp? Then you could put a nice piece of art above the fireplace.

The husband doesn't want the TV moved from over the fireplace.

I'd really love to do built ins in the future as well as add molding to the top of the fireplace to the top of our ceiling. This is on my 'future projects' wishlist.

For now I think what I'll do is paint the tile surround a dark grey (thanks for this idea!), add a plant to the left, and add a piece of metal art/cabinet to the right. Will also do a gallery wall on the perpendicular wall- here is a mock up- I'm not great at this but you get idea. I would get rid of that black console table. What do you all think?

It looks like you should easily be able to do a tall lamp behind the chair, if you want to add height to the walls. You could also add a plant to the other side or a small side table with some artwork over it. Here are some ideas from our designs which might help you more.
