It looks to be in too large of a pot with a heavy peat based soil.

basically, it's been drowning in there.

Repotting into a smaller pot with a soil that retains less water will help. You'd also want to prune all rotted roots.

id also highly recommend not using decorative pots that have a smaller opening like that. It makes repotting a nightmare.

I did have it in a larger pot & just replanted it into this smaller pot last week. I repotted it because it was looking awful & I knew it's current conditions just weren't working. I've had it since last summer & it was doing fine until this past fall & has slowly gone downhill since.

When I replanted it, there was hardly any roots to it & all the soil right around the roots were hard as a rock. I could barely break up the soil. It started looking better for a few days, but when I got home from being gone over the weekend it had took a turn for the worse.

a turn for the worse means it went into free fall and died?
