With a 42" vanity, you'll have room for a basket next to the toilet. The toilet itself isn't 33" wide; that's just the clearance needed for a person to be comfortable. A scale in the guest bath? It reminds me of the Hitchhiker's Guide to the Galaxy, particularly this: The fabulously beautiful planet Bethselamin is now so worried about the cumulative erosion by ten billion visiting tourists a year that any net imbalance between the amount you eat and the amount you excrete whilst on the planet is surgically removed from your body weight when you leave: so every time you go to the lavatory there it is vitally important to get a receipt.

If 61" is the length of the countertop, that leaves half an inch on both sides that needs to be covered with a backsplash. Tile is 1/4" thick, so you'd need to use the counter material. There will be a 1/2" gap in front of your counter on both corners. It could look funky. It would be better to get a 60" vanity and have a counter templated to fit wall to wall. Then use 1" filler strips on each end that match the vanity.

Your best size is probably 16 to 18 inches. There are about a billion of those online. Restoration Hardware has some excellent choices, well priced, and in multiple finishes. So too does Pottery Barn.RH and PB below at 16/17 inches respectively
