How to seal big gaps in concrete / plywood subfloor

Subfloor / joists / pier rotted out in corner of pier and beam house (under a shower stall). Concrete had been poured over the collapsing bathroom at some point. Hammered out the concrete in the corner and replaced pier(s) joists and subfloor. I didn't do the best job and there are gaps between the old concrete and plywood 'patch'.


What would be the best method to fill these gaps with the goal of spreading self-leveling underlayment and affixing new flooring (probably tile).
