How to fix the mess before I paint?

I was all set to run out and buy my Perma-White for the bathroom when I realized I'm not sure what I'm doing. There isn't mold, but the texture of the walls is a mess. The walls seems to be painted in high-gloss, and the painter used a roller which (in areas)left a painted texture. There are also blotches of spackle that were never sanded smooth that are now painted. The high gloss paint was applied unevenly. So... how do I deal with all this? I know I need to use Dirtex and patch and sand, but do I need to sand to completely smooth? Would a primer help? (I was planning on just using Perma-white.) Should I just get myself a heavy duty sander? I appreciate any help!
