What's replaced in a replacement window?

I've read alot of posts about replacement windows but I am still confused on what parts of a window are actually replaced?

In replacing an old wooden doublehung with a new doublehung, is it typical to replace only the sashes that move up and down, and the inside stops, and leave it at that? I would like my window to look the same from the outside of the house as it does now.

In my neighborhood, when someone replaces an old window with an aluminum slider, or a vinyl slider, any wooden exterior trim around the window is gone and never replaced. I don't know if this is necessary or if the owner just didn't like the exterior trim.
