Uh oh, I think I shoulda pruned

Usually, I stick plants in the ground, and whatever survives was meant to be. This year, I decided to be a "gardener." Turns out, there is more to growing tomatoes than I previously noticed.

I have two huge Opalka tomatoes that I thought could share a cage, for some reason. They look so small when you buy them. I must have thought one would grow out one side, and the other would go the other direction. They are 5' high and monstrously healthy, but they are busting out of the cage. The outer vines are falling toward the ground, exposing inner vines that are packed into the cage and loaded with fruit.

What can I do now, having done no pruning earlier in the season?

I was going to add more stakes outside to keep the branches off the ground. I thought the general "opening up" of the plant might actually be a good thing. Air circulation and such.

Is that a good plan, or should I sacrifice some major branches. If I need to cut - inside or outside branches?
