A few pests destroying my garden, I have pictures, any advice?

My California poppies are being decimated by these brown spots. The plants seem to grow alright, some of them even bloom pretty well for a while. Then these brown spots start out at the bottom of the stems and work their way up.

I'm going to replant some of them now, as I planted this late last year and they were really blooming well in August. (This year I planted earlier). Does anyone know what causes this, and if overwatering has something to do with it? Is there something I can mix into the soil when I plant this time that might keep it from happening?

Also, I planted these:

I can't even remember what they are, but there are some sort of worms I think getting to them that burrow. Any suggestions as to what sort of spray I can use?

Other than that, the bachelor buttons, toadflax, lupine, and scarlet flax seem to be doing alright.
