Shrubs won't grow even after 4 years

Hi, About 4 years ago we planted 4 boxwoods, 3 dwarf azaleas, and 2 dwarf rhododendrons in front of our house. The soil was tilled and prepared properly and once or twice a year I add some fertilizer. In 4 years the shrubs have not grown from their original height and actually look like they are slowly dying. Yesterday I decided to dig one up and found that the root structure is still in the same shape as the 2 gallon container that it came in when it was first planted. It appears as if the roots never ventured into the surrounding soil. I'm assuming that this is probably the case with all of the shrubs. What can I do to get the roots to expand? Do I need to dig each up and somehow loosen the root structure? Any help would be appreciated. Thanks Adam
