I've always wondered - is this normal?

The pictured lawn was set from sod ~3 years ago. It is a type of tall fescue called "Dwarf Enduro Sod" from this place: http://www.greenfieldsturf.com/productinfo.html

Overall, it's doing pretty well, except for some partially bare spots, pictured (note it's varied lighting, so the spots may not quite come out as they should).

1) What do you think might be causing the bare spots? The water coverage is OK. It's been like that for a year plus.

2) Are the tall "shoots" that come up like crazy this time of year a desirable thing (i.e. it's the actual grass making seed to help itself stay in good health)? Or have I got some kind of weed in there?

I mow on my 2nd most highest setting, and water 2x a week for an extended time.
