The latter. Rob, my guess is they’ll contact you for correction rather than flat out reject. If they catch it. Might hold things up a bit; hopefully you’re not in a rush. Again this is just a guess.

Do you mean application Kicked back? I'd say yes. I went through life as "boy" on my birth certificate for school, army, social security sign up, all was fine as a "boy" but when I applied for an enhanced driver's license "boy" wasn't good enough. The DMV told me I wouldn't be able to get a passport either as a "boy" and needed a corrected birth certificate to leave the country.

I noticed that it was somewhat more difficult to get the enhanced driver's license than it was to get a passport or passport renewal, and I had a valid passport with me when I applied for it.

rob333, did you put your mother's name in place of your birth name instead of your own name?

I need to go through the MN dept. of health and make a formal request with supporting documentation they've listed on their website to correct the birth certificate.

An example of best clarification syntax: The passport app asks for the mother's surname at the date of the applicant's birth.

The application asks for the mother's exact name, first, middle and last, at the time of their own birth, NOT the applicant's birth (and now you see why I gave the wrong answer).

Really? That application wording should be re-phrased, IMO.

I would read it as what was the applicant's mother’s legal full name when the applicant was born.

So do you have any experience with a submission now that you know the answer to your question? Have you had an application rejected or slowed down?

Do you have a copy of the exact wording of the question that you can share with us? Is it online and if so, link? Why would they want your mother’s maiden name if that was not the surname she was using at the time of your (applicant’s) birth?

“At parent’s birth” is underscored on the form. I understand making the mistake, but the form is not ambiguous.

I had not seen the wording on the form. As it is written (^^^ per foas above), it is very clear and specific.

I’m sorry for your mishap with the form, rob, and I hope everything gets resolved quickly and to your satisfaction. I am not fond of forms and paperwork. As some people her have pointed out, there are likely a lot of simple errors made. Best of luck and I hope it doesn’t stress you. If you are planning a vacation trip, I envy you and hope you have a wonderful time.

Her mom's maiden name vs married name. Rob put down mothers married name instead of her asked for maiden name. Has nothing to do with her moms given names.

If you have a passport office near you and if you have a tracking number for your application they may be able to fix it for you during the processing.

There are possible name changes other than a woman taking on a married name. I know a guy who has legally changed his first and middle names twice, and he’s only in his 30’s. All names requested are ”at parent’s birth.” The request is clear.

The form asks for names at birth and date and place of birth. That suggests part of the application procedure is to crosscheck the applicant's parents' birth registrations.

With the wrong last name, the corroborating match may not be possible. What happens then, I guess one would need knowledge of the inner workings of the passport agency to know.

Thanks petal! I have a girlfriend who has decided to become a snowbird in Mexico, and wants me to visit. She has loved it from day one and I sure wouldn't mind going to the beach. So it will be a little bit before I get down there, but probably Autumn of this year.
