Can you identify fan / how to repair?

I have a fan which I believe is likely Hunter or Hampton. It has a fluorescent light, and when it was checked out the person said it was the ballast that is bad. The light part is dead. (Edited for clarity).
I know the ballast is bad from previous person looking at it, but they didn't say model etc and wanted close to $1000 to replace it. Geez... Glad the quote was near free..
I think it can be repaired by using the following kit and covering it to LED, but need expert advice as I can't do it myself, I'm disabled.
