Preparing soil for new perennial garden

I have an area along my fence that I plan to turn into a perennial garden. It appears to have been a garden at some point- there is no grass and the soil seems somewhat better than our typical red clay, though it's fairly rocky. We took out a few overgrown junipers from one side of the bed, but the other side has been empty for years, aside from weeds. How do you suggest we prepare the soil? Should we add compost and topsoil? Do we need to try to remove smaller rocks?
