Pleasant looking room. You might experiment with different colored bulbs for your ceiling/spotlight and find a light that could be more of a whole room light than either your spotlight or ceiling fan without being s glaringly bright when you don't need that much light. There are floor lamps with two lights -- one lights up the ceiling (conversation lighting) and the other is flexible/adjustable (reading light). @ Mike Lipsky- sorry im just seeing your question. we love our fireplace in master bedroom. sorry im out of town on business so dont know height of oir fireplace but i do know we’re very happy with height we selected. i do know when we were considering what height for fireplace, we took into consideration view of fireplace when seated in bedroom chairs as well as when in bed so we could see lovely gas fire flames. would love to see a picture of your installed fireplace

When you say gas insert, to you mean a metal box you insert into the fireplace, or some type of gas logs or gas burning fire. Inserts are necessarily going to decorate the fireplace, rather they are made for efficiency. Gas logs and other gas burning effects can be more decorative, but less efficient. So it might be helpful to clarify which you are planning on installing...and if you have already chosen the gas appliance, it might help to post pictures of it since it will make a big difference in how the overall treatment needs to be done. Here's some examples of a gas insert, gas logs and gas burning fire. Example of gas insert. Notice all the black metal that is necessary to close off the fireplace. This is not a decorative solution, but more for efficiency. This is an example of a gas log with a screen and andirons. This will better preserve the look of the fireplace and provide more decorative appeal, but less heat. Gas burning twigs and black reflective glass. This is a very unique gas burning effect. Not considered an insert, but a gas fire.

The only thing that seems off is the hearth tile. Can't envision the rest of your space or the home style, but I'd replace the hearth (floor tile), with a small solid slab...maybe black granite (could get a remnant for that). Keep it simple...brick, black granite and traditional surround. The mantel and surround is classic and I'd leave it if it fits with the style of the home.
