Soil looks like a very dry peat. Do you water it? How often & how much? It is probably not getting enough light.

Thanks for the information, I'll look Kananchoe care up. I don't water very often because every time I water the leaves seem to turn yellow and fall off. When I do, I put probably 1/2 - 1 cup of water. I keep it in a west window with indirect sunlight.

That peat may be hydrophobic - totally dry and hard. It is pulling away from the sides of the pot. Roots may not be getting any water, regardless how often & how much/little you water.

You didn't say what yo are planning on using to repot. If you are getting bagged soil, get a bag of perlite. If you can get pumice easily, that would be great instead. Sift it using a kitchen sieve to get rid of the dust & smallest particles, and mix 50/50 with soil. Plant needs good drainage (all succulents do), and good light as mentioned.
