Installing car window regulator

The driver side window had a bent regulator so I replaced it. I followed instruction from a "YouTube" video. There was nothing complicated about it the new regulator I put in didn't last very long before it also was bent and the window would not go up straight. In installing it there was no question as to how it was supposed to be installed, two holes in the glass with brackets attached. I have another regulator but I don't want to install it until I get some answers. Did I miss something obvious when I intalled it? As I said, it seemed pretty straight forward.
