Can you make your own duct reducers/transitions?

I need to go from a 10"x6" opening to a 12"x4" opening.. and can't find any reducers/transition pieces that exist. Is it possible to build my own and/or any other ideas? :)

The existing hole in the wall is 10"x6" but I am putting a cabinet in front of it and am installing a toe-kick vent, where the opening of the vent is 12"x4" with a flexible duct connection that connects to another 12"x4" plate for the register..
