The problem that I see with a Goodwill shirt, is that I will not be able to find a thin cotton or mostly cotton shirt. It would be clingy and hot if it's synthetic! It will probably have cuffs too!

I also have extra long arms, it's hard to find those! The sleeves on my work uniform are too short, and they measured me! I complained, they said I would have to live with it!

You should maybe look at the scrubs that hospital and dr's office staff wear. That sounds just what you're looking for if you can find them in long sleeves. Otherwise, just hire someone to make you a few of just what you want. I do a little sewing here and there and a shirt like that would not be too hard to make, attaching the arms would be the trickiest part but for someone who does that sort of work it's probably easy.

Scrubs sounds like the perfect solution. They are light weight too. Is there a uniform store near you? If not google or amazon.

Wertach, keep looking at all thrift stores. Some are better than others. You might also ask on FreeCycle. Cut the collar off above the collar stand. If fraying, you can use zig zag stitch. (Get a decent used sewing machine ;) )

Here are some links that might be helpful. What you seem to want is a Henley or Nehru style collar. He lets can be made looser and in cotton broadcloth or linen instead of the traditional knit fabric.

Note the link for the Civil War period costume shirt. Perhaps you can find someone to sew you up a few or you can learn. I take it that you're not aiming for Brooks Brothers.

Don't count out thrift stores such as Goodwill. I get all my white, woven, long sleeve XXL or XXL 100% cotton garden shirts there. Mine have collars which I think is advantageous as they act as "perspiration radiators". But it's true you can just cut the collar right ff, leaving the band below intact and you won't have to do any sewing

Years ago I could rarely find secondhand clothes in my size, but as the US public has gotten bigger and more people own excess clothes, it has become fairly easy to find my size. I do see sizes larger than I wear, but I'll admit there are not nearly as many.

A tip for finding the kind of shirt you want at GdWll: Skim the racks and spot the white shirts. Then look for the shirts that hang very low toward the floor, as those will be the largest sizes. They will stand out from all the S, M, L and XL. And you can walk right over to them.

Or you could get a men's kameez / kurta tunic from a store that sells clothes from India, or a men's dishdashe, or short kaftan, or long sleeve dashiki. If you find a good sleeve length but want the sleeve to secure at the wrist, you can add elastic.

The other thing that occurs to me is that you could make or have made a white cotton tunic of the to that uses two to three rectangles sewn together. These are super easy; there are measuring instructions and sewing instructions on the internet. http://www.amtgard-wl.com/library/howtos/how2tunic.pdf as an example. If you hired someone with a machine and only a little bit of experience. I think they could sew up a cotton tunic for you in 20 minutes, possibly less.

I personally use these shirts for gardening / mowing. They stain pretty easily, but they're UV protective and lightweight and really comfortable. maybe duluth trading dh buys shirts from there in big/tall sizes and i've seen collarless shirts advertised....a bit on the pricey side sometimes.
