Cocktail Citrus Problems Zone 9a

I have a citrus cocktail tree which I planted back in April. The soil is
a mixture of potting soil I bought, and clay + sand which is natural to
my yard. On top is a 1/2 inch layer of Cyprus mulch. I applied
fertilome 19-10-5 citrus fertilizer once at the beginning of each season
as instructed by my nursery.

This problem originated about a month ago, the leaves start with yellow
spots and yellow until they fall. I haven't applied fertilizer since
September, and the Meyer Lemon node seems to be affected the most. Any
help is appreciated!

This is the beginning stages of the problem, it originates on the entire branch.
