Lawn leveling/grading - what to do?

I need to level/grade my front yard. I've attached several photos below. From these photos, I'm trying not only to give an idea of the type of slope I have on my front yard but also show how a sewer pipe, tree roots and a sprinkler head are clearly above ground more than they should be due to loss of soil over the years.

What is the best way to go about fixing this as well as to level everything?

Is it possible to start a lawn leveling project in the fall?

Considering that my front lawn needs to be raised at least 2-4 inches in some spots (maybe all over), would leveling need to be done with all sand or a sand/topsoil mix?

Any advice is appreciated. Fyi, grass is Bermuda 419 and currently mowed at the lowest setting on my rotary. Morpheus, in case you read this, my grass looks fantastic this year compared to years past thanks to your review of my soil sample. It actually looked even better a month ago before the Dallas, TX area got hit with the heat.
