Help! Cattleya ALWAYS root rot after repot

Hi guys,

First of all, please don't yell at me...I know it is always my fault, I really do try, and I do my research too! So, please offer some help!

So...for some reason, I always have a problem with cattleya repot! EVERY time I get a new cattleya and repot it, it will ALWAYS rot most of it's roots (if not all)...

-it doesn't matter if it is spring or summer or fall when I repot. I try to wait when new roots are JUST starting.

-I use a coarse large bark (orchiata large) and lava rock mix.

-I water once a week indoors (75-80 degrees, western window)...

-I use superthrive...

-I use clear pot with lots of ventilation holes...and I don't water unless all sign of condensation on side of pot is gone (which is usually every 7 days)...

-I pot the orchid with its rhizome just visible and on the same level as the mix...

-what am I doing wrong? I MUST be doing something wrong. The plants eventually starts producing new roots and growth...but that significantly henders the plant's vigor...
