NEC 300.3(B) and bath fan/light/night light or heater

I'm a GC that had more than a few fan/lights installed in baths on my jobs and I thought I knew how to wire until I read "Electrical Wiring Residential 15th Edition, Ray C. Mullin." On page 423, Unit 22 dealing w/ Bathroom special purpose circuits cites NEC 300.3(B) to note that it is a code violation to run two separate NM cables (one 3 wire and one 2 wire) to the light/fan/+3rd function either night lite or heater. The author claims that since 3 ungrounded conductors are needed to power the 3 functions, it must be wired via Conduit w/ indiv. conductors or 14/4+G or 12/4+G.

I've seen many such light/fans/+other function wired w/ two separate NM cables from the switch.

Anyone know a source for 14/4+G? I've seen 12/2+2 that is mainly used in place of 12/3 multiwire circuits.
