refinished floor problem

I wanted to put a higher gloss ( from satin to semigloss) on my hardwood floors than was applied about 4 years ago. I sanded and cleaned the floor then applied my first coat of semi-gloss. Let it dry for 24 hours and then lightly sanded, cleaned and applied the second coat. The floor looked great. I decided to add a third coat. This time I did not sand but cleaned it well and applied the 3rd coat. It looked great for a few days. I started to notice what looked like a scuff mark, so I decided to repair it. When I started to sand it, I realized it was not a scuff mark, but a line where the poly was not adherred to the floor! Since that first "scuff" mark, many more have appeared and now I need to resand and redo the complete floor. Could someone (Micheal?) tell me what I did wrong so I won't do this again? Thanks so much. This was my first major refinishing job and I was sooo proud of it! Oh well, hopefully the second time (with your help) I'll get it right!
