Urinary Tract Infection in Cat?

A friend of ours inherited her sister's cat when she passed away a few years ago. "Beep" is about 10 years old and he is fully clawed. He does NOT like to be handled.

Lately, our friend says that when Beep urinates, he hisses, like he's trying to scare off another cat. Being that she's a nurse, she's pretty sure it sounds like a urinary tract infection, but I think that me trying to help her get the cat into the vet would be much like putting my arm in a paper shredder.

Is this serious enough that I should just put on steel mesh gloves and get the cat into the vet, or is there something over the counter, or on 1-800-PET-MEDS she can use? Is there any way to diagnose this herself? Thanks in advance.
