Flat Chocolate Chip Cookies?

I made a batch of choc chip cookies this afternoon and a lot of them turned out flat. The baking soda was fresh and I added 1/3 cup of flour instead of the 1/4 cup that the recipe called for.

What causes the cookies to be flat instead of plump? I use butter instead of shortening. I don't know if that would account for it.
