Stubborn exterior mildew questions

OK. So here is the summary of my situation:

I have rough sawn cedar clapboards that were stained about 5 years ago now with Pratt and Lambert Solid Stain. They looked great for close to 2 years.

Then I got mildew in spots, ESPECIALLY on the sunny sides of the house. I assume the sun is breaking down the stain and providing a foothold for mildew.

Now, the frustrating part. I have tried bleach, detergent, and water solutions, as well as Jomax (following the directions) the past couple of years to try to get rid of the mildew.

Right after use, things look great. But, after a few weeks the mildew returned, but looking slightly different. What was a few number of darker circles (more pronounced), is now a fainter smear of mildew over the whole surface.

My technique was to use a garden sprayer to apply the solution and a hose to spray the solution off.

Do I need to scrub or pressure wash surface after?

Did the mildew get bleached, but not die, and re-establish itself?

Do I just need to do it again? (this is no easy task, so I want to do it right)

I am sure I need to stain again at this point, I have had significant fading as can be seen by the original color under the shutters.

Does adding mildewcide to exterior stain work, for future coatings?

This is pretty frustrating because mildew on yellow paint looks horrible...
