A friend printed out a list of her meds and other info to carry with her in her wallet. she said it was a form you fill out on www.prevention.com (a magazine site).

I've been all over the site tonight and can't find it, anyone know what I'm talking about?

I just type mine out on the computer, name, amt, Dr. Name etc. Also many clinics carry small booklets you can fill in with the medical information and they are free. Unless a person has very few meds, carrying them in your wallet may not work. Please don't forget to list the other vitamins and supplements also.

I seldom get cold, I can sleep through the nightly train that runs through town at 2 am two blocks away and the light doesn't bother me either. I'm short, so the bed is always long enough, but my big travel issue is the bed. I swear that every time I book a motel they quickly arrange to send the same lousy hard bed. I doubt that I can pack a mattress, LOL. Of course, I still sleep on my old king sized waterbed, leftover from my "hippie" days. Elery says when I grow up, I'll get a big girl bed, LOL. I hope not. Clare, on another level, I wat taking some glass to recycle last week and there was a broken picture frame in the bag. Of course, the shard stuck through the bag and into the outside of my left knee. Not one bandaid or other thing in the Jeep except black electrical tape. So, I pulled both edges of the puncture together and taped it with black electrical tape and then drove to the Emergency Room. They know me on a first name basis, LOL, and now they'll talk about that black electrical tape forever. Two stitches later that electrical tape stayed on better than their surgical tape! BEWARE OF Petsuppliesnet.com, I purchased Advantage for my cats and at the low price I purchased several boxes seeing as I have three cats. The website appear legit as mentioned above and the picture was the same as the US version (along with weight specs), but upon receipt of the Advantage, the weight, active componenets, etc were all in metric. The product was in fact from Austria. I immediately contacted the vendor to get a return number, package the items back up and mailed them back. Now they claim they never received the return and I am having to battle it with my credit card. BEWARE OF THIS DIRTY COMPANY THAT LIES AND CHEATS!

Our home is furnished in antiques. One piece was obviously made for somebody with money. It has two hidden compartments. I'd defy anyone to find those spaces! The construction of this piece of furniture is just amazing. They are large enough to hold a considerable amount of "stuff". This type of furniture, while not common, is also not uncommon. Thieves want to get in and out quickly...not hang around six hours trying to figure out if each piece of furniture has hidden compartments. I don't usually keep any real valuables around the house but sometimes I may have a special piece of jewelry around for a few days before it gets returned to its permenant residence or more cash than normal for some weird reason. I use those hidden drawers for temporary storage. The only problem I have with it is that if something should ever happen to me or DH...nobody would ever locate those compartments. This thread reminded me that I really must remember to physically show one of our adult kids how to operate the darn thing the next time they are in town. Anyway, if you live in an area with a quality antique broker...and have a need for secret storage in your home...pay the broker a visit & tell them what you're looking for. It may take awhile...but these pieces do turn up occassionally. It's obviously not an immediate solution to the OP's problem/situation but is a longer term answer. An added benefit is that pieces of this nature are almost aways real quality. So, not only do you have the usefulness of the hidden storage (and whatever else the piece is designed for)...it will probably appreciate over time & be a good investment. You are not likely to find a piece like this at your local antique mall. You'll need a really good antique broker. I've also seen a couple show up at high-end antique auctions over the years. I remember one in particular...it was known that it contained hidden compartments (3) but the auctioneer did not know how to operate them. At the pre-auction previewing...dozens of people were pouring all over that piece of furniture trying to find those compartments! If anybody did...they kept it to themselves but it sold at a pretty price on the auction block! That particular piece was a dining room table so you just never can tell by a quick glance what might have mysterious construction. We used to have some extraordinary craftsman in this country. My doctor prints one out every time I go when they go over my meds with me so that I have the latest current list to carry with me. Before that I kept a word doc on file that I kept updated and would print out for my purse.
Plus I do wear a medic alert bracelet with a lot of info and phone numbers on it.

For years I have kept a brightly colored sheet, with all health info on it, on the fridge. We each have one which has name, address, phone #, Dr. name, "as of" date, all meds, allergies, prior surgeries and current serious ailments and it is updated on the computer as changes occur. When paramedics came for DH, they asked for history, etc. I said 'its on the fridge". It was exactly what they needed and I didn't have to hunt it down or print it out. Also keep a copy in the glove compartment and a smaller one in our wallets.

Also, when DH sees any dr., he takes a copy in to attach to their updating questionnaire. His is too lengthy to have to write out each time.

I do pretty much the same as Wantoretire, does. On our fridge we have our meds list plus Dr's name, clinic, pharmacy and several contact names. HUBS & I each carry the same list in our wallets. I update it abt every 3 months, and so note it on the list the update date.

I used to keep one in the fridge. Read someplace, that this is a good place to keep it, in case of a fire, it won't burn up right away.

Yes, I keep an updated list on my computer, but haven't printed one out since I moved here, cause I don't have a printer.

Thanks for reminding me. Will print it out next time I am at my daughters.

thanks for that info. I put mine in my wallet behind the information card and live a section sticking out with E R on it. I am also going to make up a do not resuscitate red folder and hang it in a conspicuous spot in my home.

I wrote it on a business card blank and have one in my glove compartment, one in my wallet, one in DH's wallet. I have a few extra copies in a file box in case I need them.

I have also made an entry on my Ipod contacts called basically "Dances' Medications" and I list the names and dosage info there.

I do have typed out on the computer all my medical info, I print it on a 4x6 index card and update it regularily.
On the fridge at home in Michigan I have a red plastic case called Life Alert or some such thing with our medical info, I got that from my diabetes group that meets at the local hospital once a month.
Unfortunately, I have a whole lot of health problems and take a lot of meds. Just looking for a site I can type stuff into and print, then go back into to change things as meds change (which happens frequently).

You do not need a ready-made form to list your medications. All you have to do is to line up all the medications you are comsuming. Place the prescriptions in one group and supplements in another group, and then make a list using the data on the bottle labels. Put the prescriptions in the first list. Separate the next list with a couple of blank lines and proceed to list the supplements.

Each entry should contain: Drug name; (Generic name, if available, in parentheses); Drug strength per capsule usually in milligrams - include the units for example, milligrams or micrograms; Dosage; and prescribing doctor's name and a reference phone number.

Use a word processor or sread sheet and store the file under a name you will not forget, or can easily find using a "file find" untility.

On the form at top, make a header having a version number of the file and date. Each time this file is changed, also change the version/revision number and date and store the new file under a new name reflecting its creation date.

House keeping: Once a year, review the files and delete old ones that no longer serve a purpose.

I keep a copy of a similar list in a container with my prescriptions. Then, each month, I sort my pills (prescriptions and vitamins/supplements) into 'morning' and 'evening' pill holders. On this list, I separate them into three categories, 'morning', 'evening', and 'both'. Then when I'm ready to put the pills in the pill holders, I sort them into the three groups and it makes filling the pill holders so much easier and quicker.

I have a copy of the list on my computer, and carry a copy in my purse.

Years ago, I started a form for each of us, after being a girl scout assistant leader. I basically followed the overnight emergency medical form for my purposes. It has our:
Name
Address
Phones
Birthdate
Contact info in case of emergency
Insurance carrier but not numbers
Dr.'s and their numbers
Allergies
Surgeries
Medications
Supplements

I have one for each of us in each glovebox. I also have it in my notes in my Iphone, set up the exact same way. I have it in the computer, too. I've done it for my parent's also. I need to update mine. And I also need to get a bracelet or necklace. Just added penecillion to my allergy list!

I taped a card to the back of my driver's license with the most important info. It says I'm ASTHMATIC, that I have two HIP IMPLANTS, that I am ALLERGIC to Aspirin and Sulfa, MD's name and phone, DH's and DS's names and phones.

I think people would look for ID if I were unconscious.

I have my Health Insurance card right behind my license. I want the emergency folks to know they'll get paid!

If you use obe pharmacy for all your meds (recommended) , ask them to print out a list of all your prescriptions. I got mine, was surprised it went back to 2009. and it included sruff like a periodontal rinse from my dentist. There was no charge for this. I've made several copies to give to all my doctors.

Kathy g in mi - search for medication tracker. Apps to keep track of all your meds including reminders so you don't forget to take them at the desired time. Several are free - at least one of which gets good reviews. If I remember correctly you can track for multiple people presumably family members - including pets. Between me and my cat I'm about ready to resort to this to keep it all straight.

I'm not a drug addict so I don't have need for a list (vitamins, aspirin & naproxen occasionally) but after looking at the online one it made me chuckle. Let's just say I'll be forecasting frost and ice warnings in hell before I'll put all that personal data on the internet!
