estrogen along with progesterone

I am 56 and have been in menopause for a while. I still have my uterus and everything else down there. I have had no trouble with any kind of menopause symptoms besides being very "dry" down there. My dr put me on a very small dose of estrogen cream to use that would thicken things up down there after a while. My question is if I am taking estrogen shouldn't I be taking some progesterone too?
Dr said no since I am not pg and I dont have any other symptoms besides the dryness which the bioidentical estrogen I just got on should help. Couldnt afford the premarin so am on Estriol 1mg. But don't you balance the estrogen w progesterone? I don't understand this hormone business at all. Please help me.

It is not recommended to take any kind of hormones unless you have a very serious problem. There was a study years ago and the participants were women doctors. The women stopped the study and would not take anymore hormones because strokes and heart attacks resulted. Very dangerous stuff, especially if you have taken them before and start again. That is why women don't have to have paps test after a certain age because the body no longer makes the "dangerous" stuff. Dangerous was my GYN's term.

It is my understanding that bio identical estrogen is usually dispensed with some progesterone. But I question your doc for suggesting Premarin in the first place. This is the stuff we are supposed to stay away from.

You are better off taking the bio identical hormones instead of Premarin because that is NOT a substance that is found in our bodies like the bio identical stuff is.

Expanding on what others have said, the estrogen creams and gels for skin, as opposed to vaginal, use are prescription forms, made up by a compounding pharmacist to a prescription written by your own doctor (the pharmacist can help him with that if he is unfamiliar with how to write this kind of prescription). There are compounding pharmacists in Canada and they can make up these forms of HRT, but they are somewhat harder to locate (less common) than in the US. The main difference between using a topical cream/gel and using a patch is that the patch gives a steady release over the life of the patch (more or less) where the topicals give more of a curved effect, peaking after the dose and then dropping off till the next dose. Whether or not that makes a difference to any particular woman seems to be an individual thing: some dose with topicals once or twice a day and do fine; others have to go to multiple doses to avoid too much variation in hormone levels. The vehicle (there are a number of different cream bases, not to mention the gel) and where you put it (fatty tissue versus not) also affect the length and shape of the absorption/circulation curve. So, as with everything else menopausal, there's a lot of personal variation and you may have to experiment some to find the best one for your own body. There are, as Leigh notes, no effective over the counter variations on estrogen creams and gels.

The idea is that progesterone is the raw building material for a number of hormones, one of which is testosterone. In peri-menopause, our testosterone production drops dramatically. Testosterone is one of the key hormones influencing libido (but not the only thing involved in sexual response). Some of us can compensate for its decline; others cannot. The situation is worsened by taking estrogen, in a way, in that estrogen increases levels of a protein that binds testosterone. When you have fully-functional ovaries, these things balance out: you make enough testosterone to still have enough left after the protein gloms its share. When your ovarian production goes down, as in menopause, and you take HRT that contains estrogen, you are in effect hitting the testosterone with a double whammy: lower output and the protein to skim off what you do have. By adding progesterone, some women find they are able to manufacture enough testosterone again to regain their libido. Others, in whom the libido loss is due to some other problem, may not find progesterone helpful in that regard (although it may or may not be in other ways). Generally speaking, there's no way to tell whether it will help or not without trying it. But oral progesterone should work for this as well as the cream does. There are women for whom oral progesterone is ineffective--either because they do not absorb it or they too readily get rid of it--and for them, a trial of the cream form may be useful to see if it's route-related. The main difference between oral and cream seems to be that the oral route produces more sedating metabolites; otherwise, they should work the same way.

Bi-est and progesterone cream

About a 1 1/2 months ago my doctor increased my bi-est cream to 2.4 (2 clicks in the am and 2 clicks in the pm), progesterone pills (200 mg) 1 at night remained the same. I was also taking DHEA 10 mgs in the am. My DHEA level was extreemly high. All other levels were within normal range. My hair started thining, weight gain in the stomach/hips and internal itching from scalp to legs. Took a saliva test and doctor took me off the DHEA. Hair still looks thin, itching has continued and the weight has not come off. (side note - I exercise 4 to 5 times per week). Went to see a dermatologist. He said hair loss has stopped even though my hair still looks thin and he gave me pills for the itching. I believe the hair loss/thinning, weight gain and most of all the itching is due to the bi-est. I have only had one ovary removed. Can you shed some light as my doctor is not listening to me. Monica

So if I am just using a small amount of cream estrogen for my dry vagina problem do I also need to take progesterone too. Even if my only menopause problem is the dryness? I keep hearing take as little hormones as possible but I also hear that they should be balanced so what is right? My dr told me I did not need progesterone since i am in menopause and have no other menopausal symptoms except dryness. Does this sound right?

Texasone, the rationale is that the dose of estrogen delivered by these creams is so low and local that progesterone is not required. I googled "conjugated estrogen cream progesterone" and that seems to be the answer on at least several sites that came up (I've linked one, the abstract of a journal paper, below).

Hormones taken orally were the ones involved in the studies stopped early, not hormones in creams applied topically.
