tell me about the 'under cabinet trim piece' - flush or indented?

I am curious about the trim piece that goes on the underside of the cabinets (to hide the undercabinet lighting, etc.)

I like the look when the trim piece is flush to the cabinet door, as opposed to "indented" when you look at the cabinet, which is (I suppose) flush to the cabinet face itself.

To see the difference, go to the "show me your mini-brick backsplash please" thread, below. An example of the "flush" look I am talking about is shown in babka's photos. The "indented look" is shown in photos posted by, um, I can't remember the poster's name, a little farther down under babka's.

I don't know how to get the flush look! Can anyone explain it to me? Are the trim pieces just ripped down filler or toe kick pieces or do they need to get specifically ordered?
