The online site for the magazine has this Source directory for that feature: http://www.westerninteriors.com/resources/resources27.doc It states: "Page 131: Chandelier from Formations, 310-659-3062. If that's a dead-end, email the designer; the Source page has his email (and address and phone numbers). Came up empty on Google with "Formations," tho--which tells me it's either a new company or one that's to-the-trade-only. Good luck!
