replacing 1978 heatilator

Hi,

I'm planning on replacing an old heatilator that has never really worked well, and need advice. Do I need to rip out the entire steel unit and , in effect, "build" a brick fireplace before installing a wood burning insert? We would like the finished product to look nice in addition to helping with the heating bill.
