The top bracket that mounts to the wall is actually attached to the source for the original shower head. I'm not sure what your point is about your linen closet not having an access panel. It has nothing to do with new construction. I also wanted to maintain my existing tile. My home was built in the late 50's and it did not have an access panel in the linen closet either. That didn't stop the plumber from busting a large hole in the plaster wall in the closet. I created my own access panel to cover it up. Yes, we would have a hand held instead of a tub spout. Today I found the Danze Custom Shower System Design Guide states the "D113000BT valve with trim allows for supplied water to be diverted to either a single tub filler or additional shower components". I can call Danze and ask if a hand held shower qualifies as an "additional shower component" although I don't know why it wouldn't.

The hoses all seem to be about the same length. Keep in mind that the connection for the hose must remain accessible, as the hose will probably wear out at some time and need to be replaced. You might consider having the hose attach point about 3' above the tub rim and put hand shower mounts at both rim level and at shower head height. This should give you maximum flexibility.
