Is the recommended R-value for the cavity or the total area?

I suppose this is a smart question (as opposed to my usual stupid questions), but when an R-value is quoted as being recommended, is it supposed to be for the interstud cavity, or the net (which includes the studs)? The reason this is pertinent is that although studs take up about 10% of the area of a wall/roof, because the R-value for the stud is low, it has an inordinate impact on the net R-value.

For example, 3" of closed cell foam (6.5 R/in) would result in R = 19.5. However, with 10% stud area (1.4 R/in), the net R drops down to 14.3. This is a small distinction that seems to not be mentioned at websites, so I am wondering if the recommendations were done with this offset in mind. IOW, when the recommendation is for R-19, it is for R-19 in the studs, which would result in R-14, which is really what is recommended.
