Whats the potential for leaks in accessible tiled showers?

I'm not familiar with the correct terms, so I apologize in advance for any errors.

We will be building a new bathroom that is handicapped accessible (roll-in, no ledge shower). I've read the messages about Kerdi, especially Mongo's fantastic tutorial,and will try to find someone to install the system. But we need help. There aren't many examples around here of accessible showers that aren't the prefabricated kind. Our design is based on the North Carolina's publication and on the Federal Register requirements (because we don't have any other examples to go by).

So here's my question: Do tiled (built-in) benches and grab bars have a risk of leaks? That is, do either of these things that will be under stress break grout/thinset/Kerdi and risk leakage? If so, does preventative extra caulking reduce this potential? Are there any other precautions that need to be taken because of wheelchair or walker stress points? Or pulling on grab bars?

Minimum long-term upkeep is required.

Any suggestions are appreciated.
