Night Blooming Cereus problem

Hi Folks,

I'm having a really tough time with my Night Blooming Cereus . It use to bloom every year but has not for the past five. In fact it's starting to waist away.

I resides in my south facing sunroom year round. It is currently in a 16 inch terra cotta pot with regular potting soil. Transplanted it three years ago.

Leafs are browning at tips and stems then die. Out of desperation I took some ccuttings and tried to propogate new plants. Now the new crop is starting to exhibit the same problem.

Am I watering too much or not enough? Am I over fertalizing or under? Is the lighting wrong? Any help would be greatly appreciated.

Here's a shot of one of the leafs and new cuttings starting to show the same problem.
