need help with Jade tree

I have a Jade tree that I bought at a garage sale about 5 years ago, it is in a 12" plastic pot and always has been in the same pot since I've owned it (and probably long before that). In the time I've had it, it was kept outdoors until 6 months ago when I moved into an apartment where the tree gets indierct only.

At first, it appeared to be thriving in the new setup -- new leaf growth has been tremendous. But, leaves and small branches have begun falling off it with some regularlity. The top few inches of soil are very dry, but my water meter shows it as still moist mid-way down the pot.

In the photo you can see light spots where some branches fell off recently.

I'm thinking maybe it needs to be repotted? Any thoughts?
