Help with repeat ramblers

I have Snow Goose and Malvern Hills which are ramblers that repeat flower throughout the season (sort of). What is the protocol for pruing those in the winter? Also, do I have to dead head those during the summer. This would be a problem because the plants have gotten so big, I can't reach the flowers to cut them off. In the winter, I can get the ladder out and prune, but during the summer, there is so much growth in my yard, it is hard to get a ladder out there. Help!
