How to tell if I'm burning the roots?

Hi everyone,

What are the signs of burnt roots due to over-fertilizing a Phal?

I've had a Phal for almost a year now, and it is struggling. I think I'm not doing very well by it, but I'm not sure what I'm doing wrong. Over the summer it put out two new leaves and it keeps putting out new roots, but after a while the roots sort of shrivel up -- they get brown discolorations and grow less and less firm, until they are dead and hollow. I've tried to think about the usual suspects:

- Light: plenty, maybe too much. The second leaf that it put out (in late summer) has a purple blush, its color is lighter than the other leaves, and it's shorter (it's stopped growing). But the leaves aren't burnt or anything.

- Humidity: about 50%, often more.

- Watering: I'm fairly certain this is not the problem. The one root that's buried in the potting mix looks great; it's the aerial roots the Phal keeps putting out that are dying on me.

- Fertilizer: I follow the weakly-weekly plan, but I think I haven't been putting enough emphasis on the "weakly" part :-)
