Can you identify this plant and help diagnose the problem?

So there are two parts to this:

1) What plant is this?

2) What is wrong with it?

I've owned the plant for around 6 months. It lives indoors and gets a good amount of natural light. If I put a probe into the soil it is registering as '4 - moist' with a PH level of 8. The plant initially did really well but the last couple of months it has started to shed leaves like no tomorrow. It starts with the leaves curling on the tips and acting almost as if they are drying out before dropping - yet they are not near a heat source and the soil is moist. Also note the small green shoot that has appeared on one of the branches?
